Welcome to the Montana Health Centers

The State of Montana Benefit Plan serves its members and participating public employers by providing Montana Health Centers focused on high-quality, coordinated primary care to improve health and increase access to cost-effective services. Operated by Premise Health, the centers offer integrated primary and behavioral health care, along with health coaching to support your journey to a healthier lifestyle. Montana Health Centers are available to eligible plan members on the State of Montana, Montana Municipal Interlocal Authority (MMIA), and Montana University System- Helena (US-Helena) benefits plans.

Health Center Locations

Premise Health DOES NOT accept walk-in appointments. Premise Health can NOT see members for Worker's Compensation injuries. Please go to mypremisehealth.com or call (855) 200-6822 to schedule an appointment.

Please allow up to 72 hours for medication refill requests once received from your pharmacy.

*State of Montana Benefit Plan Members be aware-- Regular benefits (copay, deductible, coinsurance) apply for:

  • All MRIs and CT Scans
  • All visits to specialists (even if referred there by a Health Center provider)
  • All medications

Eligible members can view available low- or no-cost services based on their Health Plan* on the Services page.

The Montana Health Centers (operated by Premise Health) do not accept walk-in appointments.


Register for a MyPremiseHealth Account

  1. Go to mypremisehealth.com.
  2. Click “SIGN UP NOW.”
  3. Follow the prompts to setup your account.

A spouse, domestic partner, or dependent 18 years or older on your plan must create their own account and make their own appointments.

If you cover a dependent under 18 years old on your plan, you must request access to their chart in order to make appointments for them.

  • Log in to your MyPremiseHealth account, click “Menu” in the top left-hand corner, select “Request Minor Chart Access,” enter the requested information and click “Submit.” This can take up to one business day to complete.
  • Call (855) 200-6822 and schedule the appointment for your dependent. When you arrive for your appointment, request portal access for your dependent(s).

Schedule an Appointment

  1. Once you have registered for a MyPremiseHealth account, login to your account.
    • If prompted, choose yourself or a dependent under 18 you have previously registered (see above).
  2. Click “Schedule an Appointment” at the top of the page.
  3. Select the appointment option that works for you. Click here for details on the visit types.
    • Health Screening:
      • Select "Biometrics Screening." Do not make an appointment on a Provider's schedule.
    • Health Coaching: Select "Wellness Coaching," then the location that best fits your needs.
    • Premise Health Group Presentations: Select "Health Seminar," then follow the directions under "Register for a Premise Health Group Presentation," below, starting at number 4.
    • Flu vaccinations: Select "Flu Shot." Then select the Health Center location closest to you.
      • Do not make an appointment on a Provider's schedule.
      • Flu and COVID vaccinations may be administered at the same time if you have a Health Center provider order for the COVID vaccine. Applicable at a flu shot clinic or a Health Center provider visit.
    • COVID vaccinations: Select "COVID-19 Vaccine." Then select the Health Center location closest to you.
      • COVID and flu vaccinations may be administered at the same time.
    • Note: When you schedule an appointment, the system will automatically adjust the length of your appointment based on the reason for and complexity of your visit. If you are unable to find the appointment type you need for scheduling, contact a Montana Health Center for assistance.
  4. Select the location.
  5. Select the provider, date, and time for your appointment. Use the search criteria on the left side of the screen to filter available options.
  6. Verify your information and schedule your appointment.
When you schedule an appointment, this appointment is for you. A spouse/domestic partner or dependent need to have their own appointment.

Register for a Premise Health Group Presentation

  1. Once you have registered for a MyPremiseHealth account, login to your account.
  2. Click “Schedule an Appointment” at the top of the page.
  3. Select “Health Seminar.”
  4. Select the Seminar title you wish to attend, and click the orange time button underneath.
    • If the class is full, no date/time will show. Contact a Montana Health Center to see if more slots can be opened for that particular class.
  5. In the "What is the most important thing you want addressed during this visit," enter the main thing you want to get out of attending.
  6. Verify your appointment details and click “Schedule."
  7. A link for joining the seminar will be provided prior to the date of the event. Links will be sent through MyChart on the Premise patient portal.

At Your Appointment

  • Bring a valid photo ID and your insurance card to your appointment.
  • Plan to arrive 5 to 10 minutes before your appointment time to complete the check-in process.
  • If you are running late, there is a 5 minute grace period for a 15 minute appointment and a 10 minute grace period for a 30+ minute appointment.

Cancel an Appointment

  • Call (855) 200-6822
  • Login to your MyPremiseHealth account. View your upcoming appointment(s) on the home page. Select “View Details” next to the appointment you wish to cancel or reschedule then reschedule or cancel that appointment as applicable.

Virtual telehealth appointments are offered in partnership with Virtual Primary Care (VPC).

  • Access a VPC appointment with a board-certified physician from anywhere in the United States via phone or video.
  • Appointments available 24 hours a day, 7 days a week, 365 days a year.
  • Common conditions that can be treated during the VPC appointment include cough, fever, sore throat, stomachache, headache, earache, cold, flu, COVID, allergies, nausea, vomiting, diarrhea, or general skin concerns.

How to Schedule a Virtual Primary Care Visit

VPC is available even for those who don't live near a Montana Health Center.

  1. Login to mypremisehealth.com
  2. Click “Get Care Now”
  3. Enter your current location then follow the prompts
  4. Click “Schedule" and recieve notification when a provider is available
