Welcome to the Health Care & Benefits Division

The Health Care & Benefits Division (HCBD) is a division within the Department of Administration. HCBD manages the State of Montana Benefit Plan (State Plan) which provides benefits (ie. medical, prescription, dental, vision, life) for State of Montana Employees, Retirees, Legislators, and Survivors.

If you were eligible for State Plan benefits during the 2025 calendar year, you may request a paper copy of your IRS Form 1095-C.

Active employees and retirees may login to the benefits enrollment system to download a digital copy of their IRS Form 1095-C. Terminated employees and retirees must request a paper copy.
