Your mental well-being affects your behavior - how you think, feel, and act. It impacts your emotional and social well-being. Mental well-being can change over time, depending on factors such as workload, stressors, and work-life balance.

Your mental health is part of your overall health. It is important to care for your mind proactively. Achieving a state of mental well-being includes being able to:

  • Work productively
  • Cope with life events
  • Contribute meaningfully to one's family and community

If you have access to the Montana Health Centers and have a diagnosis of hypertension or are experiencing high blood pressure readings, the Montana Health Center has resources for you!

  • The Premise Health Care Team will work with you and your Primary Care Provider to develop a personalized care plan.
  • If you use a blood pressure cuff, it can be synced with the MyPremiseHealth portal to share your uploaded blood pressure readings with your provider to help guide your care.
  • Members who are able to sync a cuff will recieve:
    • Quick follow-up for any concerning readings.
    • Ongoing encouragement and education.

  • Call (855) 200-6822 and ask for a health coaching appointment;

  • During an appointment with a Montana Health Center provider, ask to be referred to a health coach.

Premise Health Coaches are available for telephone or video conference appointments in all health centers. Face-to-face appointments are available upon request.

Health Coaches

Premise Health Certified Health Coaches offer their expertise to guide you to a more fulfilling lifestyle through behavior change, mindset shifts and new habit formation while providing education to keep you more fully informed. The following are just a few of the areas in which they are subject matter experts: tobacco cessation, stress management, exercise, corrective exercise, chronic pain management, healthy eating, specialized diets, diabetes and pre-diabetes management and prevention, family/youth/infant nutrition, and weight management. Meet the Coaches

Michelle Deitsch RD, LN 

Michelle is a Registered Dietitian, Licensed Nutritionist, and Certified Health Coach.  She has experience in a variety of clinical and non-clinical areas of nutrition, cooking, intuitive eating, and diet management for patients with genetic disorders.  Michelle holds a bachelor's degree in Nutrition and Dietetics from Montana State University.  She is recognized through the American College of Sports Medicine as a Certified Exercise Physiologist (ACSM EP-C).  She takes a common-sense approach to providing support and education on all aspects of wellness.

Jacque Currie 

Jacque is a National Board Certified Health and Wellness Coach as well as a Certified Diabetes Education Specialist.  She also holds certifications as an ACE Certified Personal Trainer, Weight Management Specialist, and CDC Lifestyle Coach for the Diabetes Prevention Program.  Jacque holds a bachelor’s degree in Health and Human Performance from the University of Montana.

Jacque is passionate about helping people be their healthiest, happiest self by making incremental changes to habits.  Jacque enjoys acting as a support and partnering with people on their wellness path.

Kim Pullman, RD, LN

If you have questions about Wellness Programs available to State of Montana Benefit Plan (State Plan) members? Do you need health coaching, but don't live near a Montana Health Center?

Kim is the Wellness Program Manager for the State of Montana Wellness Programs, Live Life Well.  She is a Registered Dietitian Nutritionist (RDN), a Licensed Nutritionist in the State of Montana (LN). She is a past-Chair of the Eat Right Montana (ERM) Coalition and previously the Newsletter Editor for the Montana Academy of Nutrition and Dietetics (MTAND). Kim is certified to teach BodyPump, BodyAttack, BodyFlow, LesMills Core, and yoga.  She loves leading classes for State of Montana employees and local gyms and creating relationships with her class members.

In her position as Wellness Manager, Kim manages the State of Montana Live Life Well Wellness Program for more than 32,000 State Plan members. The program includes health screenings, healthy living challenges, presentations with wellness experts, health fairs, and a variety of condition management programs. She also provides health coaching for individual State Plan members.

Previously, Kim worked with Federal and State Child Nutrition Programs through the Office of Public Instruction.  She also worked with early childhood programs.  She has a rich history of partnership and relationships with a variety of stakeholders in Montana.

Kim welcomes opportunities to partner with other organizations to promote the health and well-being of all Montanans.

Premise Health Coach Standards

At Premise Health, we hold our health and wellness coaches to the highest of professional standards. All of our health coaches come qualified with NCCA accredited certifications and approved by the National Board of Health and Wellness Coaching to ensure you receive the highest level of expertise, guidance and evidence-based coaching. Our commitment to your health is reflected in the level and diversity of our expertise as we continue to grow our knowledge base and increase our coaching tools.

A health screening is a blood draw and biometric measurement. Staff will draw your blood and do a height,weight, and blood pressure measurement. You may also request to have a waist circumference measurement. These will be performed as directed by your Health Plan. All personal information is confidential.

Health screening benefits

  • Provide a baseline for you and your provider to monitor your health.
  • Detects "silent" issues such as high blood pressure or infections.
  • Save you and your plan money on services that are more expensive through another source.

Appointments are required. Please schedule now to ensure you get the date and time you prefer and to ensure staff has sufficient time to prepare for your appointment.

State of Montana Benefit Plan (State Plan) members: A State-sponsored Health Screening includes measurements as described below at a Montana Health Center or off-site event. A State-sponsored health screening is a no-cost benefit available to ALL State Plan members the age of 18 and older (including Medicare-eligible retirees) once per State Plan year.

  • Live Life Well Incentive for State Plan Members: Part of earning the Live Life Well Incentive for 2027 is completing a State-sponsored Health Screening between November 1, 2025 and October 31, 2026.

What to Expect at the Event

  • Health Screenings are provided in a private, comfortable environment. If you have any concerns, please communicate them to the health screening staff when you check in.
    • You will need to complete the Incentive Authorization Form once every 5 years.
  • State-sponsored Health Screenings take about 10 minutes.
  • Please do not bring children or other guests unless absolutely necessary.
  • Staff will draw your blood, do a height and weight measurement, and take your blood pressure.
    • You may request that staff measure your waist circumference. This measurement is optional. Visit the National Heart, Lung, and Blood Institute website for more information on what waist circumference and BMI tell you about your health.

Get Your Results

  • For health screenings completed at a Montana Health Center or a Montana Health Center off-site event:
    • Default: Online-only delivery — Full report and results loaded into MyPremiseHealth.com within 7 days of a State-sponsored Health Screening.
    • Opt-in: Online and mailed delivery — Full report and results loaded into MyPremiseHealth.com within 7 days of a State-sponsored Health Screening AND the snapshot (2-page summary) will be mailed to the member within 10 days; the booklet (full results including graphs) will be mailed to the member within 30 days.
    • If you brought lab orders with your to your screening from an outside provider (not a Montana Health Center provider), those results will not be included in your health screening results on the Premise Health website. See details for accessing outside provider lab order results in the Lab Tests section, below.

Lab Tests

Included with your State-sponsored Health Screening is a Lipid Panel, Comprehensive Metabolic Panel (CMP), and Complete Blood Count (CBC).

On-site screenings (at a Montana Health Center)

In addition to the three panels listed above, you may request to have most lab (blood) tests added at the same time as your State-sponsored Health Screening if you bring a prescription/provider's orders with you* to your on-site health screening. For off-site events, please review the off-site section below.


Off-site screenings (locations other than a Montana Health Center)

In addition to the three panels listed above, you may request the following three tests if you bring a prescription/provider's orders with you* to your off-site health screening. No additional tests can be added at off-site events.

  • Prostate-Specific Antigen (PSA) test;
  • Thyroid-Stimulating Hormone (TSH) test; and/or
  • Hemoglobin A1C (A1C) test.

Outside provider lab orders

You may bring orders from an outside provider if needed. An "outside provider" is any provider that does not work at the Montana Health Centers (operated by Premise Health).

  • Lab orders from an outside provider MUST include a fax number, otherwise the health center staff will not be able to process the lab order.
  • Results from outside provider lab orders completed at a Montana Health Center will NOT be included in your health screening results or on the Premise web site.
    • Outside provider lab orders completed at a Montana Health Center prior to November 19 must be viewed through LabCorp instead of the Premise website. LabCorp will also share the results directly with the ordering provider. Click here for directions on how to access your results on the LabCorp portal
    • Outside provider lab orders completed at a Montana Health Center November 19 or after must be viewed through the Billings Clinic Portal. Billings Clinic will also share the results directly with the ordering provider. Click here for directions on how to access your results on the Billings Clinic Portal

Health Screening Do's and Don't

  • Health Screening Do's
    • Do drink plenty of water in the 24 hours prior to your screening.
    • Continue to take prescription medications. If you have a condition that makes it difficult to fast, please consult with your provider before fasting.
    • Do wear clothing with loose fitting sleeves to the screening.
    • If there is any reason why it may not be appropriate for you to fast, please contact your Health Plan for assistance.
    • It is okay to brush your teeth prior to the screening.
  • Health Screening Don'ts
    • If you cannot attend your scheduled appointment for any reason, please be courteous and cancel your appointment as soon as you know you won't be able to make it.
    • Don't attend your appointment if you are sick.
    • Don’t eat or drink food or beverages other than water and black coffee 12 hours prior to your screening.
    • Don’t exercise strenuously 12 hours prior to your screening.
    • Please do not bring children or other guests unless absolutely necessary.

Eligibility

  • State Plan Members: Employees, spouses, retirees and dependents on the State of Montana health plan age 18 or older are eligible for one State-sponsored Health Screening per State Plan year. This program is voluntary.
  • Montana Municipal Interlocal Authority (MMIA) Eligibility
  • Montana University System - Helena (MUS-Helena) Eligibility

2026 Health Screening Schedule

The lab provider for the Montana Health Centers changed from LabCorp to Billings Clinic effective November 19, 2025.

Most standard, non-specialty lab services can be provided at a Montana Health Center with no out-of-pocket cost for eligible members. If you have questions about whether the lab work you need can be completed please contact the Montana Health Center.

  • Lab orders from a provider (either a Montana Health Center provider or an outside provider) are required in order to have any lab work done, with the exception of your once-yearly benefit for standard State-sponsored Health Screening labs.
  • If you do not have provider orders for the labs you need, schedule an appointment with a provider to get a lab order. Once you have provider orders for the lab work you need, make an appointment for lab work at a Montana Health Center location.
  • If you have lab orders from an outside (non-Montana Health Center) provider, schedule an appointment and bring your provider orders with you to your appointment.

Outside Lab Providers

An "outside provider" is any provider that does not work at the Montana Health Centers (operated by Premise).

Results from outside provider lab orders completed at a Montana Health Center or State-sponsored Health Screenings will NOT be included in your health screening results or on the Premise web site.


View Your Results

  • Lab results are generally available in the portal within 3-10 days, depending on the test.*
  • You can view your lab results directly within the My Premise Health web site or app.
  • Log in at mypremisehealth.com or open your MyPremise app and select “Test Results.”
  • If you need assistance or do not have access to the Internet, call (855) 200-6822.
*Please note that a delay in lab results may occur if there are abnormal values that need to be addressed by a provider.

Share With Your Provider

  • You can share your results electronically from MyPremiseHealth using the “Share Everywhere” feature.
  • Billings Clinic will also share the results directly with the ordering provider.

Please allow up to 72 hours for medication refill requests once received from your pharmacy.

Note: State of Montana Benefit Plan Members be aware-- Regular benefits (copay, deductible, coinsurance) apply for: all MRIs and CT Scans, all visits to specialists (even if referred there by a Health Center provider), and all medications.

If you are injured at work, please do not go to the Health Center.

Montana Health Center providers can make referrals for most general radiology services, such as X-rays and ultrasounds at no cost to you.

X-rays and Ultrasounds

There is no out-of-pocket cost to the member for x-rays and ultrasound referrals made by Health Center providers if the following are true:

  • Radiology must be ordered by a provider at the Health Center and
  • Radiology services must be performed at one of the radiology partners listed below

CT Scans and MRIs

State of Montana Plan Members: Regular medical benefits will ALWAYS apply for CT scans and MRIs, even if the health center refers you for these services. Please get proper prior-authorization through BlueCross BlueShield of Montana for CT scans and MRIs.

Partner Group Members: there is no out-of-pocket cost to the member for CT Scan and MRI referrals made by Health Center providers if the following are true:

  • Must be ordered by a provider at the Health Center and
  • Services must be performed at one of the radiology partners listed below.

Outside Referrals

The Montana Health Center does not accept orders from non-Health Center providers in an effort to receive no-cost radiology. A Montana Health Center provider must order the imaging services as described above.

Influenza (Flu) and COVID Vaccinations

  • Schedule online, on the MyPremiseHealth app, or by calling the Montana Health Center at (855) 200-6822.
  • To schedule a flu vaccine online, log in to mypremisehealth.com and select “Flu Shot”  or "COVID-19 Vaccine" as your reason for visit.
  • Do not schedule an appointment on a provider schedule if you just need a vaccine.

The updated 2025 influenza (flu) vaccine is available at the Montana Health Center.


COVID

The updated 2025 COVID vaccine is available at the Montana Health Center. The FDA recommendations for COVID vaccines changed starting in fall 2025. Your personal health needs will determine your eligibility for the COVID vaccine. You can discuss this with the provider during your COVID-19 Vaccine appointment.
