Montana Health Centers Disease Management
The Montana Health Centers, operated by Premise Health, offers Disease Management programs to State Plan members, such as:
- Cancer
- Peripheral artery disease
- Blood Pressure Resources
- Cardiovascular disease (including heart disease, arteriosclerosis, atherosclerosis, and stroke)
- Diabetes
- Chronic respiratory diseases (asthma, COPD, emphysema)

The Premise Health Wellness team include healthcare professionals such as physicians, mental health care providers, physician assistants, nurse practitioners, nurses, licensed counselor social workers, dietitians, fitness experts and other wellness personnel to give you the best overall care.

My Health Navigator
The State Plan has partnered with My Health Navigator (previously Hometown MTm) to offer its members assistance managing prescription drug medications, asthma, and diabetes. This pharmacist-led, no-cost service, helps State Plan members identify the safest, most effective & least costly medications, control health issues, and navigate a complicated healthcare system.
