- This option is available to active plan members, Non-Medicare eligible retirees, spouses, and/or domestic partners.
- Choose a telephone, virtual (with video), or in-person visit.

Call Blue Cross & Blue Shield of Montana at (888) 901-4989 if you need help finding a Participating Provider. - This benefit is limited to twenty (20) visits in a plan year if a member has not received training and education previously. In subsequent years, members who have exhausted the initial twenty (20) visits may have up to twelve (12) visits of follow-up education per plan year. This is the total benefit, a new diagnosis does not restart the visit count.

The Wellness Program Manager can also assist with questions about which health coaching option will work best for your needs.
- This option is available to all plan members including Medicare-eligible plan members.
- This can be a telephone or in-person (in the Helena area) visit/consultation.
- Call (800) 287-8266 or email benefitsquestions@mt.gov to get in contact with the Wellness Program Manager.
A health screening is a blood draw and biometric measurement. Staff will draw your blood and do a height, weight, and blood pressure measurement. You may also request to have a waist circumference measurement. These will be performed as directed by your Health Plan. All personal information is confidential.
Health screening benefits
- Provide a baseline for you and your provider to monitor your health.
- Detects "silent" issues such as high blood pressure or infections.
- Save you and your plan money on services that are more expensive through another source.
Appointments are required. Please schedule now to ensure you get the date and time you prefer and to ensure staff has sufficient time to prepare for your appointment.
State of Montana Benefit Plan (State Plan) members: A State-sponsored Health Screening includes measurements as described below at a Montana Health Center or off-site event. A State-sponsored health screening is a no-cost benefit available to ALL State Plan members the age of 18 and older (including Medicare-eligible retirees) once per State Plan year.
- Live Life Well Incentive for State Plan Members: Part of earning the Live Life Well Incentive for 2027 is completing a State-sponsored Health Screening between November 1, 2025 and October 31, 2026.
What to Expect at the Event

- You will need to complete the Incentive Authorization Form once every 5 years.
- State-sponsored Health Screenings take about 10 minutes.
- Please do not bring children or other guests unless absolutely necessary.
- Staff will draw your blood, do a height and weight measurement, and take your blood pressure.
