New this year! You must complete four activities to earn the LLW Incentive for 2027:

MediKeeper is available for all State Plan members the age of 18 and older; however, only employees, retirees, legislators, COBRA participants, and their enrolled spouses/domestic partners are eligible for the Live LIfe Well Incentive.


Register for the MediKeeper Wellness Portal

  1. Visit the MediKeeper Wellness Portal.
    • If you are a State of Montana employee on the State network, you may access MediKeeper through Single Sign-On (SSO) by clicking the "Access the MediKeeper Wellness Portal" button above. Joint Core partners create a username and password through the Medikeeper website. They cannot use the SSO button to access MediKeeper.
    • Those not on the State network, dependents, and Joint Core partners do not have access to SSO and should visit the off-network, dependents, and Joint Core partners login, then select the grey button that says, "Or click here to register."
  2. Fill in the requested information.
    • If you are not logging in via SSO, you will need to register your account. Use your first and last name as it appears on your BlueCross & BlueShield of Montana medical ID card.
    • You must create a username and password. Although it is not required when logging in via SSO, if you skip this step you will NOT be able to access the system, even to register and create login information, anywhere other than through the SSO.
    • Accounts cannot be shared. Each State Plan member (active employees, spouses, retirees, COBRA, etc.) will need to register and create their own account.

Please make sure your email is updated and allow MediKeeper to send you notifications. This allows you to unlock the full potential of the Wellness Portal, receive reminders and communications about upcoming or ongoing Challenges, and more.

You can update your email preferences under "My Account."

MediKeeper Wellness Portal Help

  • Once you have signed in, click on “Incentives & Rewards.” You may also click the “Keep Earning More” button from the top banner on the MediKeeper home page.
  • Click “View Program” in the “Current Programs” box.
  • Click the “+” icon located to the right of the activity you’d like to complete.
  • Click on the blue "i" button if you need more details.
  • Follow the prompts.
  • Once you are done, you can confirm by checking that the activity now says (1/1) or click the “+” icon again and it will say, “Completed.”
    • The MediKeeper Wellness Portal uses "points" to indicate if a member has earned the Live Life Well (LLW) Incentive. You have completed all four required LLW Incentive Activities once you have earned 4 "points."
    • You can also confirm by checking that each of the four required Activities say "Complete" rather than "Incomplete."

Remember that State-sponsored Health Screening will appear automatically appear on the MediKeeper portal about a week after your Health Screening is complete.

You may also download and use the MediKeeper App, Alaveda, from the App Store.

  • Within the MediKeeper site, navigate to the TRACKER tile. A "tile" is a square button below the welcome banner.
    • You can also navigate the site by clicking on the pull-down menu next to the home button in the top left of the screen.
  • Click the TRACKER tile and/or "View My Tracker."
  • Click on MANAGE DEVICES.
  • Follow the prompts provided.

A note about Samsung Health: At this time MediKeeper cannot sync devices for activity tracking with Samsung Health. There are workarounds that members may be able to use to get their data to sync with MediKeeper.

  • Fitbit: Syncs activity data like steps, calories burned, and sleep from Fitbit to Samsung Health and vice versa.
  • Strava: Enables sharing of GPS-based activities (like runs and bike rides) from Samsung Health to Strava and vice versa.
  • Garmin Connect: Can be synced with Samsung Health to bring in activity data.

How to Connect Samsung Device to MediKeeper:

  1. Open Samsung Health: Go to the "More" or "Settings" section within the Samsung Health app.
  2. Find "Connected Services" or "App Connections" The exact wording may vary slightly.
  3. Select the app you want to connect: Choose the app you want to link with Samsung Health from the list.
  4. Follow the on-screen instructions: You'll typically need to log in to the other app and grant permissions for data sharing.

How can I verify that I'll be receiving the Live Life Well Incentive for teh 2026 Plan Year?

To verify if you will be receiving the Live Life Well (LLW) Incentive for 2026, review your benefit elections.

  1. Go to benefits.mt.gov. If you are on the State network, click on the "On State Network Benefit Enrollment System Access" button. If you are off the State network, click on the "Off State Network Benefit Enrollment System Access" button and login to the system.
    • If you have not accessed the system and you are a first-time user, you will need to register and create a user name and password.
    • The "Company Key" is stateofmontana.
  2. Click the drop-down arrow next to your name located in the upper right corner and select "Benefit Summary."
  3. In the "My Health" section, under "Medical," click "View Details." This will cause a pop-up window to appear.
  4. Under the "Plan Selected," it will show if you and/or your covered spouse/domestic partner earned the LLW Incentive.
  5. The election information section will show a bi-weekly (for employees) or monthly (for retirees) deduction from your contribution amount for 2025.
    • For active employees, the LLW Incentive is displayed as a bi-weekly credit because employees pay their contributions through bi-weekly payroll deductions.
      • For employees, the LLW Incentive will show as a $30 bi-weekly credit on your Benefit Summary.
      • If both you and a spouse/domestic partner earned the LLW Incentive, it will show as two $30 bi-weekly credits, one for you and one for your spouse.
    • For retirees, the LLW Incentive is shown as a monthly credit because retirees pay their contributions monthly.
      • For retirees, the LLW Incentive will show as a $60 monthly credit on your Benefit Summary.
      • If both you and a spouse/domestic partner earned the LLW Incentive, it will show as two $60 monthly credits, one for you and one for your spouse.

If your LLW Incentive is not applied correctly, contact HCBD immediately.

New for 2026! To earn a Live Life Well Incentive in 2027, you must complete the Health Assessment along with a State-sponsored Health Screening, self-reporting your Nicotine Free status, and self-reporting completion of an Eligible Provider Visit by October 31, 2026.

  • The Health Assessment is a questionnaire designed to help you identify risk factors, provide individualized feedback, and link you with interventions to promote health and/or prevent disease.
  • The Health Assessment delivers meaningful, personalized insights into your health by assessing current and potential health risks. For example, eye and hair color can help identify sun sensitivity, while lifestyle and biometric information helps identify disease risk and overall wellness.
  • The Health Assessment is entirely confidential and takes 15 minutes to complete.

Complete the Health Assessment

Start your Health Assessment today by logging into the MediKeeper Wellness Portal.

  1. Visit LiveLifeWellMT.medikeeper.com.
    • If you haven’t already, you must create an account on our Wellness Portal, Medikeeper. Enter your name exactly as it appears on your BlueCross & BlueShield of Montana medical benefits ID card. Covered spouses or domestic partners must make their own account using their personal information.
    • Once you’ve created your account, you may also use the MediKeeper app, Alaveda, to complete the Health Assessment & other Live Life Well Incentive Activities.
  2. Once you’ve logged in to MediKeeper or the Alaveda app, click on “Health Assessment.”
    • The Health Assessment will take about 15 minutes to complete, and you don’t have to do it all at once.
    • Previous year Health Assessment results can be accessed by clicking on the “HRA Yearly Comparison” button (not the “Health Assessment” button).
    • You may also access the Health Assessment on the "Incentives & Rewards" button, next to the "Health Assessment" activity.
  3. Once you have completed the Health Assessment, click on “Incentive and Rewards,” it will show as “Complete” in the “2026 Live Life Well Incentive” section.

Complete a State-Sponsored Health Screening

You must complete the Health Assessment, a State-sponsored health screening, self-report your Nicotine Free status, and self-report completion of an Eligible Provider Visit by October 31, 2026 to earn a Live Life Well Incentive in 2027.

Part of earning a Live Life Well Incentive on your 2027 benefit contributions is getting a State-sponsored Health Screening between November 1, 2025 and October 31, 2026.

  • Once you have completed your State-sponsored Health Screening, it will automatically be loaded into the MediKeeper Wellness Portal; State-sponsored Health Screenings take up to 3 weeks to be loaded into the Portal.
    • If you complete your State-sponsored Health Screening after October 13, it may not show up on the MediKeeper Wellness Portal by October 31; however, your screening will apply retroactively to your Incentive once it is loaded.
      • This is due to the 3 week delay between when your screening was completed and when it will be loaded into MediKeeper.
    • If you complete your State-sponsored Health Screening after September 9, credit for your Live Life Well (LLW) Incentive may not show up on the Open Enrollment System during Open Enrollment; however, (and provided you have completed all three Live Life Well Incentive Activities by October 31) your LLW Incentive will apply retroactively to your account. It will show during the confirmation period in early December.

Self-Report Nicotine Free Status

Health Assessment, a State-sponsored health screening, self-report your Nicotine Free status, and self-report completion of an Eligible Provider Visit by October 31, 2026 to earn a Live Life Well Incentive in 2027.

Part of earning a Live Life Well Incentive for 2027 is to self-report that you are Nicotine Free or have completed an eligible Nicotine Free alternative between November 1, 2025 and October 31, 2026.


Eligible Alternatives Available

If you use nicotine, you can still earn the Live Life Well Incentive if you complete and self-report one of the two eligible alternatives:

  • Complete(d) a nicotine cessation program between November 1, 2025 and October 31, 2026; OR
  • Have a nicotine education session with your primary care provider between November 1, 2025 and October 31, 2026.

Nicotine Cessation Programs

Learn more about no-cost nicotine cessation programs available to State Plan members. Completing a nicotine cessation program is a good option for members who are ready to quit using nicotine.


Nicotine Education Sessions

A Nicotine Education Session is an appointment with your provider for the purpose of discussing your nicotine use and possible options. This is a good option for members who may not be ready to quit using nicotine. To complete this alternative, schedule an appointment with your provider.


Definitions

  • Nicotine:
    • Nicotine is an addictive stimulant proven to have negative health effects that is found in cigarettes, cigars, chewing tobacco, and most vaping products.
  • Nicotine Free:
    • You may self-report you are Nicotine Free if you have never used nicotine, have quit using nicotine, use only FDA-approved Nicotine Replacement Therapy (NRT), or infrequently use nicotine (less than 4x per month). The State Plan offers an exception for religious/ceremonial use.
    • You may self-report you are Nicotine Free if you are currently using nicotine but HAVE completed an eligible alternative (nicotine cessation program or a nicotine counseling session with a medical provider) during the past 12 months.
  • Nicotine User:
    • You are a nicotine user and may not self-report you are Nicotine Free if you are currently using nicotine and HAVE NOT completed an eligible alternative (nicotine cessation program or a nicotine counseling session with a medical provider) during the past 12 months.

You must complete the Health Assessment, a State-sponsored health screening, self-report your Nicotine Free status, and self-report completion of an Eligible Provider Visit by October 31, 2026 to earn a Live Life Well Incentive in 2027.

As part of earning the Live Life Well Incentive for 2027, you must complete and self-report an Eligible Provider Visit. The majority of our plan members indicate that they find a annual provider check-up valuable and necessary to maintain their good health.

  • You must complete an Eligible Provider Visit as one of your three required Live Life Well Incentive Activities between November 1, 2025 and October 31, 2026 to earn the Live Life Well Incentive in 2027.
  • Self-report if you have completed an Eligible Provider Visit by October 31, 2026.

What is an Eligible Provider Visit?

  • An Eligible Provider Visit is an annual physical examination appointment with a medical provider. It is usually done in-person. Exceptions to this are very rare, as virtual visits do not normally allow the provider to adequately complete a physical examination.This is not an acute or urgent care visit (i.e. cold, flu, or injury), it is an annual checkup.
  • This type of appointment will usually take 45 minutes to an hour.
  • Any kind of medical provider is acceptable, from a nurse practitioner to a specialist, depending on who is most appropriate for your health needs. Choose a medical provider (such as a Montana Health Center provider) that best fits your needs.
  • This visit will include a complete physical examination based on your age, gender, and other factors. You and your provider will decide exactly what is included in your appointment based on your health needs.
  • This visit may include a discussion of your health screening results - it is not an appointment to only review health screening results. If your results are available, please bring them along so your provider can review the results with you during the appointment.
  • During the visit, your provider may renew your prescription for medication you take every day.
  • During the visit, be prepared to talk with your provider about any health concern or needs you have. Your provider can refer you for behavioral health, counseling, physical therapy, specialists, and more. Keep in mind that your regular benefits apply to this visit and any additional services beyond the provider’s time may incur extra expense. Not sure what to ask about? Download this handy checklist.

Remember, if you can attend a Montana Health Center for your Eligible Provider Visit, all services provided at the health center are at no additional charge!


More Information

The State Plan offers the LLW incentive program to all enrolled employees (for the purpose of this disclaimer employees includes active employees, retirees, legislators, and primary COBRA participants) and their enrolled spouse/domestic partner.

If you choose to participate in the LLW incentive program, you will be asked to complete a voluntary health risk assessment that asks questions about your health-related activities and whether you have or had certain medical conditions (e.g. cancer, diabetes, or heart disease). You will also be asked to obtain a voluntary blood test for certain conditions (e.g. cholesterol). If you choose to participate, you will receive a reduction in your bi-weekly employee contribution. If you are unable to participate in any of the health-related activities of the LLW incentive program, you may qualify for a reasonable accommodation to earn the incentive.
